Make a Difference
As a Post-Acute RN Case Manager, you will have the opportunity to make a profound impact on the lives of people living with complex and/ or chronic conditions. Some of the Post-Acute RN Case Manager duties may be performed remotely. The Post-Acute RN Case Manager may spend partial or full days at home, depending on the nature of work being performed. You may connect with patients and or their family members in Skilled Nursing Facilities or on the phone to assist in improving their health, better understand their illness, and coordinate their care. The Post-Acute RN Case Manager will coordinate care by serving as the contact point, advocate and resource for the patient and their family, building effective relationships through trust, respect and communication. In close collaboration with the patient and care team, you will continually assess the patient's knowledge of their clinical condition(s) and/or the discharge process and provide education as needed. The Post-Acute RN Case Manager will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
Exceptional Skills and Qualifications
Graduate of National League for Nursing, Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing (required). Licensed as a Registered Nurse by the Indiana Professional Licensing Agency - (required). Three years of nursing experience (required); preferably in Population Health management or Post-Acute Care Experience in case management, skilled nursing facility, home health, ambulatory care, community public health, coordinating care across multiple settings and with multiple providers (preferred). Team building skills with the ability to function independently and interdependently as a member of a network team. Demonstrates the ability to be flexible in work schedules and coverage at any site. Strong organizational and problem-solving skills. Exhibits openness to change and willingness to try new approaches to problem solving. Can establish rapport with a wide variety of people. Strong computer skills.
