**Position Summary:**
The Affinia Health Practice Coach-Office Specialist is responsible for providing direct outreach, consulting, and support to physician / provider practices to make improvements in systems and delivery of care. Their primary function is to navigate staff towards successful completion of all Patient Centered Medical Home initiatives and to coach managers, providers and staff to develop the capacity for sustained change and improvement. Practice coaches collaborate closely with offices to reach and maintain Patient Centered Medical Home Designation and facilitate quality improvement initiatives using lean methodologies in order to increase the efficiency of practices and deliver on the triple aim goals of better health, better care, at a lower cost.
Additionally, the Practice Coach-Office Specialist serves as a subject matter expert in additional content areas such as advanced care planning, HCC coding, behavioral health integration, and other key strategic initiatives. When serving in this capacity, the Practice Coach-Office Specialist focuses on program development and project management that supports population health alignment, AHN strategic initiatives, and care coordination for Affinia responsible lives. The Practice Coach – Office Specialist is also responsible for consulting and working on projects with relevant practices on an ad hoc basis, training and educating staff, and presenting project updates to leadership.
**What you will need:**
+ **Education**
Bachelor’s Degree in Healthcare or another related field.
Preferred: Clinical background preferred. Master's degree strongly preferred.
**Credentials/Licensure**
Valid Michigan driver's license and proof of auto insurance.
Preferred: For clinical professionals (LPN, RN, PA or NP), current MI licensure in the appropriate profession.
**Related Experience**
Minimum one year of PCMH experience. Completion of 2 week training with the project management of process improvement team. Three to five years of experience in a health care setting with demonstrated responsibility for process improvement and quality improvement. Knowledge of quality improvement concepts, clinical practice transformation models and methods including Patient Centered Medical Homes.
Preferred: Prior experience in practice transformation and Patient Centered Medical Homes preferred. Familiarity with change management facilitation and practice coaching preferred.
+ **Knowledge and Skills**
Understanding of basic ambulatory clinical processes of care. Familiarity with data systems, including registries. Ability to understand and explain data reports in different ways to different stakeholders. Demonstrated effectiveness in facilitation of teams and familiarity with group dynamics and communication. Strong oral and written communication skills. Strong interpersonal skills. Strong customer service skills. Good project management skills. Good presentation skills. Good computer skills, including standard office software and applications. Ability to build relationships and engage others in team efforts.
Preferred: Knowledge of other regulatory, patient safety, patient satisfaction programs, and payer quality programs. Excellent interpersonal and facilitation skills. Patient Centered Medical Home content knowledge desired.
Minimum: Ability to travel to Provider offices within a broadly defined area.
**What you will do:**
+ **Primary Focus Area:**
+ Facilitates practices in the adoption and operation of the Patient Centered Medical Home model.
+ Works directly with clinic managers, providers and staff to identify and address issues related to maintaining an effective and efficient medical home practice.
+ Acts as a knowledge broker who knows about external resources and tools and saves practices from engaging in extensive searches for information.
+ Help determine areas of improvement by assessing needs, gathering data, and guiding discussions with the practice. Provides facilitation and quality improvement coaching for Affinia Health Network practices. Under the direction of Quality leadership, assists with prioritization of work and provides process support to network practices related to quality incentives.
+ Builds and maintains excellent relationships with provider organizations.
+ Supports and coaches practices in the collection and management of relevant project data.
+ Facilitates communication with other Mercy Health staff to enhance awareness of Network issues and initiatives.
+ Responsibly stewards resources by minimizing waste, and recommends changes to reduce and/or contain costs.
+ Performs other related duties as assigned.
+ **Secondary Focus Area:**
+ Oversees relevant projects from conception through operational phases. Coordinates the assessment, analysis, planning, implementation and evaluation of complex/multi-faceted projects/ programs including providing data reports and analyses.
+ Brings together and coordinates technical, clinical, and business project team members, assign responsibilities, holds stakeholders accountable, and coordinates their work.
+ Develops all project documentation (including requirements, scope, and schedules), manages budgets, and communicates with stakeholders.
+ Develops and adheres to a standard project management methodology and quality standards; and ensures project success at every level.
+ Determines priorities and deadlines, and organizes work schedule to meet them.
