As a Practice Facilitator, you will work closely with primary care practices to build capacity for performance improvement activities and help the practice reach incremental and transformative improvement goals. In this role, you assist in redesigning workflows and processes so staff can better serve patients. While you enhance your skills, your fellow caregivers and superiors will encourage you to explore new interesting areas and collaborate with diverse groups within and outside the Cleveland Clinic institution.
A caregiver in this position works remotely from 8:00AM – 5:00PM.
A caregiver who excels in this role will:
Assess and provide feedback to practices regarding organizational, clinical, and business functions to drive change. Use practice-level and physician data to drive change. Coordinate initial data connection training and system access for all new clinical caregivers and provide initial support at the worksite. Evaluate provider proficiency at scheduled sessions and create or modify a lesson plan designed to maximize provider efficiency. Coordinate and implement the distribution of new clinical functionality to practices. Schedule group user training when necessary. Document and diagram existing workflows. Identify opportunities for improvement and work with the administrative team to design and implement these improvements. Train practice caregivers to understand and use data effectively to drive Performance Improvement in the measurement domains for their specialty. Train caregivers on quality measures and improvement methods and specific transformation processes, such as team-based care. Provide regular updates to administration on the level of user proficiency, issues and plans. Promote best practices in quality structures and methods related to performance measures. Facilitate both project and change management. Use health IT to support capacity for improving clinical care and office efficiency. Cross-pollinate best practices among primary care practices. Build capacity for improved networking with outside resources. Provide technical assistance in implementing particular models of care, such as chronic care management. Assist, practice, organize, prioritize, and sequence measure quality improvement activities and, in some cases, achieve medical home recognition. Increase practice capacity for continuous performance improvement activities and create and maintain an ongoing performance improvement infrastructure within the practice. Build a team orientation, promote effective communication patterns among practice caregivers and help create a practice culture that is receptive to change. Provide direct support to caregivers in properly using clinical information systems. Create individualized lesson plans for each provider based on the provider’s proficiency with systems such as the EMR, the results reporting system and others.Minimum qualifications for the ideal future caregiver include:
Bachelor’s Degree in Business, Healthcare Administration, or Nursing and five years of experience in an ambulatory setting OR Social Work or Counseling Certification or an LPN, MA or RN with five years of clinical experience Valid Driver’s License (VDL) Knowledge of best practices with private practice rooming intake, making the most of office visits, patient reception workflow and structure Expertise in acquiring and using data to drive improvement, and knowledge of performance improvement methods Polished presentation skills with the ability to present to a variety of levels up and down Excellent PC skills with proficiency in MS Office products and developed aptitude towards technology Ability to travel regionallyPreferred qualifications for the ideal future caregiver include:
