Practice Manager, Healthcare Transformation Program

The Practice Manager for the Healthcare Transformation Program will be responsible for oversight of clinical and financial operations for an innovative collaboration between Swedish Hospital and local Federally Qualified Health Centers (FQHC’s). Through this collaboration Swedish Hospital specialty care lines will be embedded in FQHCs to reduce access barriers and address social determinants of health. The Practice Manager will be responsible for all providers and staff, coordinating assignments to ensure effective patients flow in the day-to-day operations of patient care services at the Swedish campus and off-site FQHC locations. The position is supported by Health Care Transformation Program funding of the Illinois Department of Health Care and Family Services (HFS).

Position Highlights:

Position: Practice Manager, Healthcare Transformation Program

Hourly Pay Range: $33.66 - $54.50. The hourly pay rate offered is determined by a candidate’s expertise and years of experience, among other factors.

Location: Chicago, IL (Swedish Hospital Campus)

Full Time/Part Time: Full Time

Hours: Monday-Friday, 8am-5pm

Required Travel: travel to other clinics within the area will be required

What you will do:

Manages and oversees all support staff in the delivery of Patient Care Services.

Coordinates staff schedules and hours including vacation approval and facility coverage. 

Works with Senior Management and physicians to ensure off-site operations and relations are effective and consistent with policies and procedures.

Acts as a liaison between clinical support staff, medical staff, patients, and FQHCs

Assists in developing workflows for social determinants of health screening and clinical case management.

Responsible for financial and data management of multiple off-site locations

Responsible for managing data related to referrals, scheduled appointments, no shows and collaborating with other Transformation Program colleagues to track and improve access and quality metrics.

Participates in the development and implementation of operational and clinical policies and procedures.

Responsible for the facility management activities.

What you will need:

Education: Bachelor’s degree in the area of business, health care administration or acceptable years of experience in managing a health care center or private physician office.

Certification: n/a

Experience: 3+ years of practice management experience including at least 2 years in a health care organization. 

Unique or Preferred Skills:

Knowledge of fiscal management and human resource management techniques. 

Knowledge of office management techniques and practices. 

Knowledge of computer systems and applications. 

Skills in gathering, analyzing and evaluating data. 

Ability to apply policies and procedures and principles to solve everyday problems and deal with a variety of situations. 

Ability to exercise initiative, problem-solving and decision making. 

Ability to establish priorities and coordinate work activities.
