JOB DESCRIPTION:

SUMMARY OF POSITION             

Serves as the liaison for coordination of accessing care between The Substance Abuse and Mental Health Services Association (SAMHSA) Pregnant and Postpartum Women Pilot (PPW-PLT) program known as the Pregnancy Promise Program. Work with client to identify support professionals. Provide care coordination, connecting individuals to treatment. Coordinate with Program Case Managers to address clients’ unmet needs.

ESSENTIAL DUTIES & RESPONSIBILITIES

Provide targeted case management and care coordination. Connect individuals to treatment, recovery services and community resources.Ensure that communication occurs between the Pregnancy Promise Program Case Manager and the Centerstone Mental Health and SUD providersEnsure that communication occurs between the Pregnancy Promise Program Case Manager and Centerstone staff at times of transition with the client’s care.Work with the client to identify support professionals to formally participate in the client’s team Meetings and share the Plan of Safe Care developed with the family during their Pregnancy Promise Program enrollment.Work with the Pregnancy Promise Program Case Managers to communicate about and address clients’ unmet health-related social needs including housing, transportation, food insecurity, childcare needs, safety concerns and other unmet needs by determining which party will be responsible for making referrals, follow-up to ensure the resource received, and documenting the outcome of the referral. Collaborate with Volunteers of America Indiana and other SAMHSA grant partners to share best practices for treating pregnant and postpartum individuals with mental health and SUD diagnoses.Collect client level data known as the Government Performance and Results Act and report to FSSA.

KNOWLEDGE, SKILLS & ABILITIES

Effectively, communicate via written, verbal, in person and virtual methods.Skilled in active listening.Proficiency in MS Office (Word, Excel).An effective team player with strong inter-personal skills.Demonstrated ability to work and make decisions in a fast-paced environment.Knowledgeable about addiction, recovery and family dynamicsKnowledgeable of pregnancy and post-partum conditionsOrganizational skills

QUALIFICATIONS

Education Level

High School diploma or equivalent required. Bachelor’s degree preferred

Years of Experience

Minimum of two years of experience working with individuals with substance use and mental health disorders. Understanding of substance use disorders, medicated assisted treatment, the recovery process and co-occurring disorders required.

Required Certification/Licensure

Must become State Certified within 90 days or attend the first available State training from date of hire.
