Prenatal Care Educator - Pathways - Full Time - Days
The Educator, Prenatal Care/Community Health Worker is a home visiting position that works to improve overall health and birth outcomes with clients assigned to their caseload. The Prenatal Educator/CHW delivers health education to clients and develops pathways to better health or health improvement plans, connecting clients to vital resources. The CHW may also conduct outreach in community neighborhoods to engage the most vulnerable and underserved populations in order to connect them to preventive care and much needed resources. Other duties include, but are not limited to: Provides outreach to pregnant women and other potential clients in the community who are not already enrolled in prenatal care or who have been referred to the Pathways program. Contacts clients assigned to their caseload to schedule and conduct home visits; maintains a full caseload at all times and meets productivity standards set forth by Hospital Council of Northwest Ohio (HCNO) as well as ProMedica Ebeid Children’s Hospital. Completes all funder required and supervisor recommended education and trainings to maintain CHW certification as well as all hospital regulatory competencies annually. Education: Must have a GED or High School Diploma Skills: Must be able to input and retrieve information from the computer. Must be able to understand and follow directions, communicate clearly both in writing and orally; must be organized and respond to inquiries. License: Must have reliable transportation and be able to travel locally on a regular basis. Must have current car insurance and a valid driver’s license. Certification: CPR, BLS PREFERRED QUALIFICATIONS Education: Associates degree in related field Skills: Must be able to input and retrieve information from the computer. Must be able to understand and follow directions, communicate clearly both in writing and orally; must be organized and respond to inquiries. Effective time management skills are a plus. Years of Experience: One or more years of community outreach related experience preferred. Certification: Community Health Worker certification highly desirable.
