**_Prescription Assistance Liaison - Mercy Health Youngstown_**
**SHIFT/SCHEDULE:**
+ Full Time - Scheduled for 40 hours per week
+ Shift Times - Days/Afternoons
**SUMMARY OF JOB RESPONSIBILITIES:**
To assess, assist and complete client intakes and original applications for chronic medications for the purpose of facilitating access to the pharmaceutical manufacturer patient assistance programs for those individuals, 18 years old and above, who are un-insured and under insured. The prescription assistance liaison acts as an assistant to the program manager and is an advocate for the client the medical community and the manufacturer.
**QUALIFICATIONS/EDUCATION:**
High school diploma required
Medical terminology preferred
Pharmacology knowledge a plus
Bi-lingual skills a plus
**EXPERIENCE:**
Prior experience in Prescription Assistance Programming preferred
Medical office or hospital-related clerical experience preferred
**KNOWLEDGE AND SKILLS:**
Requires basic reading, writing and simple arithmetic skills as demonstrated through pre-hire assessment
Basic computer skills required
Ability to follow written and oral instructions required
Knowledge of Medicare Part D guidelines preferred
Knowledge of Social Security Low Income Subsidy preferred
General knowledge of local social services agencies for emergency assistance preferred
General knowledge of financial aid programs within the HMHP system and the community preferred
Utilization of Community Outreach cross-selling tool required
Ability to compose letters of communication in Word format required
Knowledge of Excel preferred

Adheres to HIPAA guidelines and confidentiality
**PRINCIPAL JOB ACCOUNTABILITIES (Essential Functions):**
Complete all demographic information in software database
Investigate all possible PAP programs for respective clients
Data enter and update software program consistently
Keep client files in alphabetical order by company name and date sequence
Executes co-ordination of applications for client within all HMHP PAP sites
**JOB ACCOUNTABILITIES – Other:**
Performs other duties as assigned by the manager or Community Outreach Director
Files charts
Answers telephones
Take accurate messages
Returns telephone calls in timely manner
Adheres to time and attendance policy
Adheres to appearance policy
Liaisons assigned to Ambulatory Care Center will pull client files from chart as needed
Liaisons in Ambulatory Care Center notify clients of receipt of PAP medications in the clinic
