Prevention Specialist

Position Summary:

In general, the Prevention Specialist functions as the primary client advocate, keeps abreast of progress, and coordinates all services delivered to the participants both internally and through external referral.

Essential Job Functions:

Responsible for insuring program participants patients receive all appropriate services; Coordinate services provided by ancillary agencies; coordinates and participates in treatment planning and reviews with the entire treatment team; Consults with each patient concerning attainment of treatment plan goals and objectives; Conducting community-based outreach activities; Provide HIV and Hepatitis testing and counseling testing and linkage; Develop Individualized risk reduction Prevention Plan in collaboration with the consumer; documenting all sessions in the clinical record; Provide HIV risk reduction counseling (using evidence-based practice); Implement evidenced based intervention; Gain certification to provide youth and young adults the evidence-based community Participate in the development and implementation of the environmental strategies; Document all service delivery in clinical record in accord with Federal, State, and agency standards; Participate in all staff meetings and training as assigned by the Program Coordinator; Participate in Performance Improvement activities; Transport clients when necessary; Work in collaboration with Program Coordinator to co-facilitate support groups and other activities; Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; and All other duties as assigned.
