Prevention Specialist

osition Summary:

The Prevention Specialist acts as the primary client advocate, keeps abreast of progress, and coordinates all services delivered to the client and family internally and through external referral.

 

Essential Job Functions:

Conduct Needs Assessment Develop Service Plans in collaboration with the client and family Conduct monthly Service Plan reviews Document all clinical services and activities in accord with licensing, Medicaid, CARF standards and agency policy and procedure Conduct individual case management sessions Facilitates access to health insurance and public entitlements. Perform activities on behalf of the client and family, including attendance at external appointments as needed Provide referral and linkage for the client and family, as needed Prepare monthly Progress Reports, as required, for assigned clients for courts, probation officers, caseworkers, etc. Conducts HIV/VH test using OraSure or OraQuick and conduct pre/post counseling using CLEAR Provides education about safer sex practices and substance use prevention Coordinate client and family care with the treatment team and with external providers Arrange transportation for clients, as necessary Participate in case conference Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation Participate in meetings and attend trainings as required and requested Completes all orientation and mandated training activities, including E-Learning Courses, within required timeframes, and in accordance with the criteria established for this position. Perform any other duties as assigned.
