Preventive Supervisor
SCO Family of Services
The Supervisor is responsible for managing a team of direct service staff to provide supportive counseling and case management to increase safety, reduce risk and promote wellbeing utilizing the Family Treatment & Rehabilitation (FTR) model of practice. The Supervisor works closely with their team, the Program Director and Mental Health and Substance Abuse Specialists to insure all responsibilities are carried out in accordance with model fidelity and the standards of FTR, ACS and SCO Family of Services. Responsibilities include:
Manage a team of 4 Case Planners and 1 Case Aide to provide assessment, goal setting, interventions and case management to prevent foster care and promote wellbeing. Learn the FTR practice model by participating in training, coaching and case consultation. Insure staff fidelity to the FTR model: Work within model structure regarding case contacts, interventions, supervision, documentation and length of treatment. Insure best practices by providing coaching, guidance, direction, support, assistance; and monitoring staff of performance. Provide individual supervision. Conduct and document monthly supervisory reviews of all cases. Work collaboratively with an interdisciplinary team including Mental Health and Substance Abuse Specialists and FTC Facilitators. Facilitate Clinical Diagnostic Team Meetings to determine treatment plans. Support staff to assess safety and risk and implement crisis management plans and safety plans as needed. Manage crises and provide case coverage as needed. Manage intake in absence of Program Director as requested. Insure staff engage in professional development as required and indicated by learning needs and goals. Manage HR and performance improvement related processes with support of program director. Collaborate with the Family Team Conference (FTC) Facilitator and insure Family Team Conferencing occurs and adheres to best practices; attend conferences when indicated. Collaborate with ACS Division of Child Protection and Family Court Legal Services to insure staff attend child safety conferences and court hearings. Utilize strategies to incorporate family voice and value of racial equity in program. Utilize strategies to develop and maintain a trauma informed environment. Responsible for insuring proper and timely utilization of all PROMIS and CONNECTIONS data base systems and preparation and submission of data reports as applicable. Implement strategies to insure staff maintain case record documentation in alignment with ACS, OCFS, FTR and agency wide case record review standards. Review all assessments, progress notes, Family Assessment and Service Plans (FASPs) and monitor PROMIS data entries. Track Key Performance Indicators on quantitative and qualitative outcomes aligned with ACS and model expectations in collaboration with QI. Attend supervision and all necessary meetings and trainings. Participate in neighborhood-based activities and preventive related advocacy, work groups and coalitions as requested. Demonstrate strong sensitivity to cultural and socioeconomic diversity. Commit to being an active contributor to the vision and mission of the program. Work to create and support a respectful learning environment. Perform all other duties as assigned.
Qualifications
Master of Social Work or related field with commitment to obtaining license. LMSW/LCSW preferred. Bilingual Spanish a plus. At least 3-5 years previous experience working in a child welfare context. Previous direct service work with families. Self-initiated, motivated and well-organized. Excellent writing and communication skills. Facility with OCFS & ACS data systems and Microsoft Office tools.Flexibility to work some evening hours to attend conferences, manage crises.
