Purpose
The Primary Care Program Manager of Social Worker will support the primary care social work team by providing consultation, training and supervision, community networking and outreach. The Program Manager will provide consultation and supervision in the standards and expectations of the Neighborcare Health’s Social Work role, and the organization’s Mission, Guiding Principles and Goals. The Program Manager utilizes his/her expertise to help social work staff members manage challenging patient cases and colleague requests, stay within their role responsibilities and professional ethics, and grow in their skill and abilities as a practicing social worker.

In this position you will:
Provide consultation to the primary care social work team in a group and individual format. This includes: Consultation for emergent patient concerns Consultation regarding effective practice and professional collaboration in a multidisciplinary medical setting Professional and ethical consultation to assure the staffs’ care is consistent with Neighborcare’s Health policies and social work role. Work with the Program Manager for Behavioral Health and Social Work to align their consultation and supervision with the Neighborcare social work program expectations and standards. Collaborate with cross-functional teams to drive systems improvement Participate in community outreach events Responsible for any other duties as assigned by the Director.Key Skills, Knowledge, & Abilities

Experience working in a leadership role with a diverse team of health care providers and administrators Experience working in a primary care setting Depth of practice experience as a Licensed Independent Social Worker providing care to patients with varied socio-economic and cultural backgrounds and varied medical and psychosocial presentations. Bachelor of Social Work/Human Services or other degrees related to the field Three (3) + years of direct supervisionPreferred qualifications
Master of Social Work/Human Services or other degrees related to the field Eligible candidates will hold a Washington State LICSW Eligible candidates will be an approved supervisor for LICSW and LASW candidates as detailed in WAC 246-809-334 Five (5) + years of direct supervisionOur Mission

Compensation
The wage range for this position is minimum $96,394.00/salary up to $137,155.20/salary maximum
