Job Summary:

Join our team as a Primary Support Professional and provide Individualized Supported Living (ISL) services in collaboration with the Program Coordinator.

As a Primary Support Professional, you'll work in the home and community with assigned clients to ensure the satisfactory implementation of their Individual Support Plan (ISP). You'll provide direct support services, coordinate daily activities, and ensure the health, safety, and cleanliness of the residential homes. Our ideal candidate has experience in the IDD field, strong communication skills, and the ability to effectively advocate for clients.

Key Responsibilities:

Work in the home and/or community with assigned clients as designatedParticipate in Individual Support Plan meetings and RN/service monitoring visitsTrain staff on client's medical needs, personal preferences, and medication complianceOversee daily residential notes, goals, and health tracking; complete required checklistsMaintain house calendar and ensure all annual medical appointments are up to dateEnsure adequate supplies of groceries, medication, and household needs; order as neededReview physical condition of home to meet health, safety, and cleanliness standardsEnsure safety kits are present and complete; review Medication Administration RecordsComplete event reports in accordance with state guidelinesSchedule staff or cover shifts for personnel who are absentTrack mileage and client expenditures; ensure proper receipts and money countsCollaborate with Program Coordinator on follow-up, training, and documentationEnsure safety drills are conducted and staff are trained on special diets/equipmentDocument daily exceptions and submit to Program CoordinatorPerform other duties as assigned

Education and/or Experience Qualifications:

Must be 18 years of age with a high school diploma or GEDOne year of experience in the IDD field or successful completion of Missouri Quality Outcomes training

Required License/Certification (one of the following):

Valid driver's license, proof of insurance, and reliable transportation

Additional Qualifications:

Ability to read, interpret, and write routine reports and correspondenceEffective communication skills with clients, families, and other stakeholdersAbility to apply common-sense understanding and deal with standardized situationsProficiency in basic math operations using whole numbers, fractions, and decimals
