As a Primary Support Professional-Residential, you will work in the homes of people receiving residential services, ensuring the satisfactory implementation of the Individual Support Plan (ISP). You will collaborate with the Program Coordinator to provide support services and coordinate day-to-day activities as outlined within the ISP. Your role will involve training staff, overseeing daily notes and health tracking, ensuring safety standards, and much more.

Key Responsibilities:

Work in the home or community with assigned clients, as designated by the Program Coordinator and/or the Regional DirectorParticipate in Individual Support Plan meetings and RN visits or service monitoring visitsTrain staff in each client’s medical needs, personal preferences, and complianceEnsure staff are knowledgeable of client’s medications and any potential side effectsOversee daily residential notes, goals, and health trackingReview physical condition of home to ensure health, safety, and cleanliness standards are metEnsure safety kits are present and completeReview MARS for signatures and errors per shiftComplete event reports in accordance with state guidelinesSchedule staff or cover shifts for personnel who are sick or absentTrack mileage used in home to stay within budget for each clientParticipate in the development of positive behavioral support if neededTake clients to all required medical appointmentsEnsure receipts are maintained for each purchase made from client’s money and recorded on appropriate ledgerEnsure money counts are completed at each shift change by staffCollaborate with Program Coordinator on follow-up to monthly RN visits, trainings for special diets and adaptive equipment, and annual paperwork signed by guardiansEnsure safety drills are done on each shift according to the Individual Safety PlanEnsure staff are trained in special diets and adaptive equipmentEnsure daily exceptions are documented and submitted to the Program Coordinator at the end of each monthPerform other duties as assigned

Education, Experience, and/or Credential Qualifications:

Must be 18 years of age; must have a high school diploma or GEDOne year experience in the field of intellectual/developmental disabilities or in lieu of experience, must successfully complete training in the Missouri Quality Outcomes approved by the Division of DD regional office

Additional Qualifications:

Must have a reliable means of communicationMust possess skills to read and understand written materialsMust possess skills to write clearly and legiblyCurrent driver’s license, acceptable driving record, and current auto insurance

Physical Requirements:

ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human bodyRepetitive movements of hands, fingers, and arms for typing and/or writing during work shiftSedentary work involves sitting most of the time but may involve walking or standing for brief periods of time
