Schedule:

Monday – Friday10 AM – 6 PM60 hours/week

Responsibilities:

Provide high-quality, individualized nursing care in a comfortable home environment.Monitor patient’s condition, administer medications, and update care plans as needed.Support daily living activities and ensure the safety and comfort of the patient.Collaborate with family members and the healthcare team to deliver exceptional care.

Qualifications:

Responsibilities Perform selected nursing procedures within the scope of LPN practiceProvide patient and family teaching according to plan outlined by the RNMonitor patient's vital signs and make specific measurements and observationsMonitor and record intake and output.
