PRN Registered Nurse Behavioral Health

As a Behavioral Health RN, you will:

Patient Assessment & Care Planning: Evaluate patients and families to identify physical, psychosocial, and environmental needs. Develop, implement, and revise individualized plans of care based on ongoing assessments.

Patient Visits: Conduct initial evaluations and follow-up visits, addressing patient, family, and facility needs.

Communication & Reporting: Notify supervisors, physicians, and team members of significant findings, changes in health status, or unsafe conditions.

Education & Counseling: Provide education on behavioral health philosophy, symptom management, and care plans to patients, families, and facility staff.

Team Collaboration: Supervise and document care provided by behavioral health aides and LPNs.

Problem-Solving: Address facility issues and report concerns to supervisors as needed.

Qualifications:

Current, unrestricted state RN licensee

1+ years of experience in a clinical care setting as a nurse

Able and willing to travel within branch/office coverage area.

Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

Must be able and willing to travel 50%

Associates Degree or Bachelors Degree in Nursing is preferred

Previous Home Health or Hospice Experience is preferred

Previous experience with Homecare HomeBase is preferred
