PRN Hospice/Palliative Nurse Practitioner
**Employment Type:** Part time **Shift:** **Description:** **Position Overview:** We are seeking a compassionate and dedicated PRN Hospice/Palliative Care Nurse Practitioner to join our team. In this role, you will provide comprehensive palliative care and hospice services, focusing on symptom management, end-of-life care, and support for patients and their families. This position offers flexibility and the opportunity to make a meaningful impact in the lives of those facing serious illness. **Key Responsibilities:** + Conduct comprehensive assessments of patients’ physical, emotional, psychosocial, and spiritual needs. + Develop and implement individualized care plans in collaboration with interdisciplinary teams. + Provide symptom management and palliative interventions, ensuring comfort and quality of life. + Educate patients, families, and caregivers on disease progression, treatment options, and end-of-life care. + Coordinate with physicians, nurses, social workers, and other healthcare professionals to ensure integrated care. + Complete documentation in compliance with regulatory and organizational standards. + Participate in on-call rotations and provide telehealth support, as needed. **Qualifications:** + Master’s or Doctorate degree in Nursing. + Current Nurse Practitioner license in the State of Michigan. + Certification in Hospice and Palliative Nursing (CHPN) preferred + Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred + Strong communication, assessment, and critical-thinking skills + Ability to provide compassionate care in challenging situations + Valid driver’s license and reliable transportation for field visits **Apply Today!!!** **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
