Pay is based on experience and shifts with a range of $28.00 to $33.00/hr. **What You’ll Do:** + Provide **comprehensive, high-quality nursing care** in a home setting under the direction of a Registered Nurse. + Collaborate in developing **personalized care plans** with an interdisciplinary team. + Educate patients and caregivers on health management. + Accept and document verbal and written physician orders, with RN co-signature as needed. + Maintain ongoing communication with **physicians, patients, families, and team members** regarding patient conditions. + Ensure **accurate and timely documentation** of all care provided. **What You Need:** + **Active LPN license** in Connecticut (or Compact License). + **Pediatric LPN experience** (minimum of 12 months). + Home health experience preferred. + Reliable transportation to fulfill job duties. + Strong commitment to **quality patient care** . + Ability to **lift, push, and pull 50-100 lbs** . + Willingness to **travel (50%)** as needed. **Pediatric Registered Nurse – Key Contributions:** + Perform **meaningful evaluations** and provide detailed reports. + Recognize and adapt to **changing patient needs** while maintaining high-quality care. + Effectively communicate patient condition changes to the healthcare team. + Lead with **understanding, respect, and appreciation** for the skills of others. + Uphold **Elara Caring’s mission and values** to ensure top-tier patient care.
