PROGRAM COORDINATOR, MENTAL HEALTH SERVICES
+ POSITION SUMMARY + Reports To: + Practice Manager + Summary: + The Program Coordinator, Mental Health Services provides individual and family psychotherapy and functions as a core member of a collaborative care team. The coordinator is responsible for supporting mental health care and coordinating referrals to clinically indicated services outside the primary care clinic. This role provides wrap-around services to support clinician practices with supervision and case management. Services provided are, but not limited to: psychosocial assessment, consultation, crisis intervention, education, community liaison, information and referral, coordination of services, and advocacy. + Work Schedule: + Full-time position includes patient-facing clinical hours and hours for clinical follow-up and administrative duties including program development, supervision of team members, and community meetings. + SKILLS & ABILITIES + Education + Master's Degree in Mental Health Counseling, Social Work, or Marriage and Family Therapy required. + Experience + Two or more years related experience preferred. + Computer Skills + Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility. + Must exhibit basic computer knowledge. Knowledge in using electronic medical records and other databases preferred. + Certificates & Licenses + Current with all state licensing requirements. Must have LCSW, or eligible to take exam and pass within 6 months of hire. + Other Requirements + Knowledge of behavioral health and addiction conditions and best practices for treatment and follow-up. Effective listening and interviewing skills to explore client needs and develop and monitor treatment plans. Excellent written and verbal communication skills and ability to document information in a timely and effective manner. Ability to work independently and to manage and prioritize tasks while also working as part of a collaborative team. Ability to build and maintain community partner relationships and establish professional collaboration with providers.
