Program Coordinator, Palliative Care-North

JOB SUMMARY

Provides overall coordination, development, facilitation and operations of the Palliative Care Program at Hendrick Medical Center. Collaborates effectively with physicians, patients, families, and other departments to promote this service line.  Works in collaboration with an interdisciplinary team in an active practice to deliver palliative care to patients and families. 

JOB REQUIREMENTS

Minimum Education Clinical Background - Graduate of Accredited School of Nursing Minimum Work Experience 2 years management experience Required Licenses/Certifications Current RN License Current Driver's License Required Skills, Knowledge, and Abilities Accountable for the regulatory requirements, function and duties Ability to carry out obligations and duties of position Demonstrate advocate of quality and cost effective care Excellent human relations and oral/written communication skills Demonstrates interpersonal skills Deals effectively with community/consumers Understand the physical, psychosocial and spiritual concerns/problems of palliative patients and families Must be knowledgeable of quality assessment and performance improvement activities Compose applicable policies and procedures Audit, monitor, and retrieve data Coordinate meetings Proven written and editing skills Strong computer skills Requires ability to handle confidential information
