For the health of our community ... we are proud to be a tobacco-free campus. **Responsibilities** Under the direction of the Program Manager of the MAPC the Program Coordinator delivers community based outreach education and recreation programs facilitates support groups and is the team lead for the pre-screening process for new movement disorders clinic patients. + Maintains current knowledge of disease, treatments and related disorders + Provides information and education to the community, patients, staff and other professionals regarding movement disorders, the MAPC and its scope of activities + Coordinate and screen patients entered into the movement disorder program's Parkinson disease database and pre-screening program for new patients, DBS patients and follow-up patient appointments + Administer psychometric tests (within score of practice) and coordinate distribution and data entry of PD scale to PD clinic patients + Attend and contribute to team meetings + Lead recreation classes as an instructor (as appropriate and within qualifications) + Plans, develops, organizes, coordinates, and conducts programs (e.g., education, recreation, and social activities) in community settings to facilitate development and rehabilitation of people with movement disorders + Contributes to the publication and distribution of bi-annual calendars (MAPC programs) to persons with PD as well as to internal/external customers + Submits articles for newsletter as requested + Assists in securing financial/funding opportunities and develops grant proposals + Reports any deficiencies and safety concerns regarding the MAPC van to Operations. Maintains compliance with Hospital + Plans, develops, and facilitates new or existing support groups + Conducts and assists in research projects + Increases community awareness and public relations relative to movement disorders Dignity Health now offers an Education Benefit program for benefit-eligible employees. Five or more years of experience in clinical care/rehabilitation preferably in neurology and/or geriatrics. Ability to successfully coordinate complete and routine processes. Ability to quickly identify problems and execute successful solutions. Ability to multi-task. Familiarity with patient-centered care and working as part of a team. Arizona license in applicable discipline Certification as necessary American Heart Association BLS CPR **PREFERRED** Skills in critical thinking effective communication conflict management problem solving autonomy and decision-making are highly desirable. **Pay Range** $30.55 - $44.30 /hour We are an equal opportunity/affirmative action employer.
