Saint Luke's Koontz Center for Advanced Breast Cancer is looking for an experienced RN to join their team as a Program Coordinator.
This position is accountable for coordinating the programmatic operations of the Koontz Center for Advanced Breast Cancer.
The responsibilities of this role include...
Participating in program planning organization and development with system leadership, hospital leadership, and the medical director. Collaborating with specialty care departments and patient care units regarding all aspects of multi-disciplinary care for advanced breast cancer patients seen within the center.Coordinating process improvement and review and trending of quality metrics for the program.Meeting with patients and families to provide emotional support, education and guidance to all patients.Providing medical information, education and review of reports and knowledge of community resources.Facilitating rapid scheduling and follow-up tracking of patients to ensure completion of their recommended treatment plan.Identifying and problem solving any service gaps identified.Promoting the Navigator role and prevention, detection and treatment awareness to the community through presentations and participation in related organizations.Coordinating and providing education for all newly diagnosed patients from our campus and outside community.Providing support to clinic nurse roles and maintains many additional job functions as needed.The Program Coordinator acts as a resource facilitating staff development and other special projects, including bi-annual metastatic breast cancer retreats.
In this role you will assist with marketing and the Saint Luke's Foundation to promote and support programmatic development. This position is also responsible for the clinical navigation patients through their diagnosis and treatment ensuring timely services.
Job RequirementsApplicable Experience:
