Program Coordinator I, Behavioral Health, SBHC

This is a full-time grant funded position through Heluna Health-Children and Youth Behavioral Health Initiative (CYBHI). Continued employment BEYOND June 2026 is contingent upon additional funding.

POSITION SUMMARY
The Behavioral Health Program Coordinator I (BHPCI) will be a valuable member of the NAHC SBHC team by providing coordination, project planning, group facilitation and administrative support for the SBHC Department’s Heluna Health-CYBHI grant across two of our school-based health center sites. The SBHC Department collaborates with three school districts to provide comprehensive health services at seven SBHCs with the mission to reduce health inequities by
focusing on prevention and youth empowerment. NAHC is seeking a bright, motivated, detail oriented individual to work closely with the SBHC Department Leads and the SBHC Behavioral Health Team to support our mission.

The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).

DUTIES AND RESPONSIBILITIES
Program Coordination
1. Will be responsible for the implementation of the Heluna Health-CYBHI Grant: Will coordinate programming, student follow up, data reporting/tracking, and the
implementation and group facilitation of Support for Students Exposed to Trauma (SSET) program to meet grant deliverables.
2. Manage program operations related to groups: Receive referrals for youth who are candidates to participate in SSET program, identifying days/times of groups, obtain sign-in sheets, purchase reimbursable program and food supplies, plan for student participation reminders.
3. Will provide one on one support to students and families to teach the basic concepts of social and emotional health by using a skills-based approach to focus on topics related to social determinants of health, depression, anxiety and trauma.
4. Will work with Behavioral Health Clinicians and Care Coordinator to provide referral and follow up for students with identified SDOH needs and positive scores on trauma screeners such as PEARLS ACES, SDOH, and PHQ-As.
5. Manage grant data tracking system such as data entry, tracking information, and scheduling activities to support with grant reporting.
• Ensure that all required data strategies, collection, and documentation of program objectives and outcomes occur in a timely manner across sites for grant
deliverables.
• Responsible for entering accurate data for grant deliverables into the Data portal and other electronic databases.
6. Engage youth through youth focused workshops, events and programming targeted to reach a larger student body on campus to bring awareness and education on different behavioral health topics. Provide support with the organizing, setting up and staffing of these events.
7. Create culturally and linguistically appropriate educational materials to engage youth and community members.
8. Engage with families and school staff in order to build knowledge and skills on trauma informed care, self-directed change and community development.
9. Educate and advocate for the development and/or enforcement of public/ institutional policies that address individual or school community needs with health providers, school staff and partner organizations.
10. Actively participate in staff meetings, workshops, and trainings; as well as coordinate activities with collaborating agencies.
11. Represent the SBHC Department in meetings, task forces, and partner collaborations.
12. Meet regularly with supervisor to ensure services are aligned with vision of the agency and in compliance with department, agency, and funding agency regulations.
13. Member Care: Greet all youth, families and school staff in a courteous and professional manner to create and maintain a welcoming atmosphere.
14. Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
15. Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
16. Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
17. HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA guidelines for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to Site Director and HIPAA security officer.
18. Work well under pressure, meet multiple and often competing deadlines.
19. At all times demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients and the community.
