The Program Coordinator is responsible for providing oversight and coordination of operations within designated service delivery areas. Areas may consist of in-home or community-based services. Be knowledgeable of the System's Policies and Procedures, and any department-specific proceduresMaintain confidentiality of Protected Health Information as directed in HIPAA trainingCoordinate and participate in the development of the Individual Support Plan (ISP); ensure outcomes are measurable and clear to team membersEnsure team members are trained in the individual's written plan and in their responsibility for implementing the planEnsure accurate and timely documentation is completed for authorized services and supportsSupervise and evaluate team membersConduct, at a minimum, quarterly Record and Therapy Reviews to ensure records are current, complete and contain substantial documentation that services were provided as authorizedMake weekly visits to the ISL to monitor the status of the individual, complete PC weekly checklist, make sure of the health of the individual, and make sure premises are safe and cleanMaintain client profile in database, enter staffing pattern and prepare annual ISL budgetsTrack monthly ISL budget exceptions and prepare end of year variance reportsEnsure all team members have current training, including any training specific to the individual and updated as changes occurWork in conjunction with Primary Support Staff and Residential Nurse to ensure medical needs are taken care of, all medicines and treatments are given as ordered by physician, and encourage wellness such as good diet, exercise, and healthy habitsComplete daily documentation audits in Therap to include health, meds and daily notes, goals and GER (General Event Reports)Coordinates the provision of medical care to those served and encourages wellness such as good diet, exercise, and healthy habitsIn residential settings, ensure money counts and controlled drug counts are being done at the beginning and end of each shiftComplete Monthly Reviews on an individual's progress or lack of progress on goals and objectives in the ISP, and overall status of the individualFacilitate opportunities for natural support and community participationOther duties as assignedEducation and/or Experience Qualifications:
A bachelor's degree from an accredited college or universityRelevant experience in the field of Developmental Disabilities can be substituted year for year for a degreeSupervisory Requirements:
The Program Coordinator will directly supervise Community Service Managers, Primary Support Staff and Direct Support Staff.
Communicate the strategic direction of the organization and encourage participation by all team membersProvide leadership and guidance to all aspects of the departmentTake an active role in monitoring the identification, development, and execution of strategic objectivesInvolve, as appropriate, all team members to achieve goalsEffectively communicate to team members any changes and newsworthy events within the department or companyHandle difficult team member situations directly, using appropriate discretion and Human Resource advice to show respect for the individualChampion change and effectively manage the implementation of new ideasReinforces team approach throughout functions; support and solicit input from team members at all levels within the companyEmployment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint checkCompletion of New Hire Orientation at the beginning of employmentAll training requirements including Relias at the beginning of employment and annually thereafterCurrent driver's license, acceptable driving record and current auto insuranceMust have working air conditioner and heater in any vehicle used to transport clientsWill be on-call according to the regional office schedulePhysical Requirements:
