STATUS:Non-exempt, full-time (37.5 hours per week)
HOURLY RATE: $21.00 to $22.00

SUMMARY
Supports daily operation of the NYS Long Term Care Ombudsman Program in Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne, Wyoming, and Yates Counties. Implements strategic objectives to achieve the program’s goals and coordinates the program’s volunteer recruiting, training, performance evaluation, and recognition activities as assigned by the Program Manager. Advocates for and educates about long term care residents’ rights in the region.
Duties AND RESPONSIBILITIES
1. Supports daily operations and implements planned strategic objectives, as assigned by the Program Manager, to achieve program goals.
i) Participates in all agency staff meetings, program update meetings, and team meetings.
ii) Participates in quarterly data audits conducted by the Office of the State Ombudsman to ensure compliance with program data reporting policies and procedures. Implements corrective measures as needed.
iii) Maintains knowledge of and applies program policies and procedures to case intake, handling, disposition, and reporting.
iv) Coordinates certification training, continuing education training, or data reporting activities as assigned by the Program Manager.
v) Collaborates with other Lifespan programs and aging service providers as appropriate.
2. Supervises volunteer ombudsmen
i) Recruits, trains, and supports volunteers. Completes annual performance evaluations for volunteers, subject to the Program Manager’s review and final approval.
ii) Monitors volunteers’ performance to ensure compliance with agency and program policies and procedures. Provides feedback and guidance when issues arise and informs the Program Manager when issues continue or remain unresolved.
3. Serves as a certified ombudsman
i) Completes the initial 36-hour NYS ombudsman certification training program.
ii) Completes 18 hours of continuing education training each year to maintain state certification.
iii) Provides direct advocacy services for regional long term care facility residents, visiting at least one facility weekly.
iv) Completes monthly documentation following program policies and procedures.
v) Meets weekly with the Program Manager for supervision and case review.
4. Performs other duties as assigned by supervisor.
Requirements
QUALIFICATIONS
EDUCATION:Bachelors’ Degree in Social Work or another Human Services-related field.
EXPERIENCE:Experience in a human service agency, health care or long term care setting preferred.

· Maintains composure under strict deadlines and difficult situations.
· Able to work in a multi-cultural environment.
· Has reliable transportation and can travel within region to assigned facilities.
· Proficient working with Microsoft Office.

If working less than 37.5 hours, accrued time is pro-rated.
