Harborview Medical Center has an outstanding opportunity for a Program Coordinator - **Patient Scheduler** to join our Rehabilitation Medicine Clinic.
**WORK SCHEDULE**
•Full-time, 100% FTE (40 hours per week)
•Day Shift Monday - Friday
**DEPARTMENT DESCRIPTION**
The Harborview's Rehab Medicine Clinic has an outstanding opportunity available for a full time (40 hours a week), day shift PROGRAM COORDINATOR. The PC coordinates all patient services such as admitting, discharge appointments, arranging diagnostic procedures, consultation requests, and scheduling services from other hospital departments in order to facilitate delivery of care to patients

**RESPONSIBILITIES**
•Coordinate complex scheduling of diagnostic procedures and consultations
•Work with Patient Services Specialists (PSS) to schedule less complex referrals as well as appointments with scheduling directions in place
•Facilitate scheduling of urgent referrals and procedures and work closely with Clinic medical director to triage
•Communicate with discharge planner and Contact Center Complex Appointing Team to schedule priority patients with various clinics in a timely fashion
•Verify outpatient benefit information for a seamless transition to clinic-based services
•Ensure completeness and accuracy of registration and patient demographic data.
•Manage a high volume of phone calls, emails, faxes, and letter correspondence pertaining to patient and/or provider follow-up, continuity of care, and referral decisions
•Collaborate with PSS Supervisor and PSS to create, update and maintain all clinic templating that meets the needs of patients, clinic staff and providers.
•Utilize system resources, including computerized information systems and staff from ancillary departments, and develop processes to meet the needs of the service and patients
•Act as primary liaison for the patient with hospital, referring provider, insurance carrier, and outside agencies and staff from other health providers to coordinate activities involved in pre-operative planning
•Assist patients with completing insurance forms and requests for information by providing documentation to facilitate coverage for scheduled visit, admission and/or procedure
•Work with partner PCC to ensure planned procedures are pre-authorized in time prior to admission and follow-up on insurance denials.
•Coordinate supporting documentation to facilitate and coordinate patient care involving agencies outside of HMC
**REQUIREMENTS**
•High school graduation or equivalent AND two years of experience in the program specialty
•Scheduling and patient coordination skills with at least 1-2 years' experience processing referrals and scheduling patients, ideally in a larger volume medical system OP clinic
•Familiarity with gender affirming care, the related coverage requirements (provider letters)

•An equivalent combination of education/experience
