Program Director, Nursing Administration

The Program Director will be responsible for leading a variety of programs across the enterprise. This will require collaboration with various stakeholders and clinicians to create programs that are in line with the organization's strategic objectives. The Program Director will oversee the implementation of an Oncology Nurse Navigator (ONN) program, which will utilize evidence-based practices to achieve transformational and value-based initiatives. In addition, the Program Director will work closely with stakeholders and clinicians to establish a Practice Transformation governance that will improve patient-centered care, patient experience, clinical quality/outcomes, clinical risk reduction, patient safety, clinical data management, internal/external reporting, and physician satisfaction.

The Program Director is responsible for ongoing development, coordination, training, and facilitation of implementation for the organization’s value-based initiatives. Responsible for the successful planning and execution of key strategic activities that will enable the advancement of value-based care operations. Partners closely with contracting, operations, analytics, clinic leadership, and care management. The Value-Based Care Director will develop strategies to meet and sustain program objectives, goals, and metrics.

 

As a successful candidate, you will:

Oncology Nurse Navigation:

Direct the development of the ONN Program in accordance with the Oncology Nursing Society’s, Nurse Navigator standards and competencies. The ONN Program will focus on prevention & and screening and will facilitate care through diagnosis, treatment, survivorship, and end-of-life care. Be responsible for the assessment and monitoring of caseload allocation & and available resources based on program metrics. Serve as the subject matter expert on the design and implementation of Navigation software and tools to support the program such as: a referral system, an acuity system, and the development of quality measures. Develop lean workflows, processes, and an infrastructure that can support the success and scalability of the program as well as the optimization of the ONN role to practice at the top of their license.   

 

Value-based Care (VBC):

Develop, implement, and direct operational and clinical activities for commercial value-based care programs, aligning clinical operations with quality metrics and enhanced care coordination. Identify and implement strategies to meet requirements of value-based care arrangements, reduce total cost of care, and enhance quality patient care. Analyze data, identify deficiencies, and generate an action plan to resolve those deficiencies. Engage and collaborate with network contracting team to create alliances with commercial payers in the development of value-based care opportunities. Align strategies for practice transformation through standardization of operational and clinical processes to meet requirements of value-based care programs. Engage with payers and vendors to assess clinical components for value-based care programs and the ability to support and/or partner to enhance performance and potential revenue opportunities related to such arrangements. Collaborate with Care Management to align and monitor care coordination activities required for participation in value-based care programs. Manage, oversee, and monitor all quality programs, facilitating physician and practice education, to ensure positive performance. Assure completion of all required data submission and documentation of participation in quality programs. Generate and archive documentation required for audit purposes for quality programs.

Advanced Care Planning (ACP):

Lead for ACP, the Clinical Lead/Program Director directs the development, implementation, and evaluation of the ACP program for City of Hope. Improve systems, strategies, and processes to increase ACP awareness and promote Advance Directive (AD) completion. Conducts strategic analysis of processes and workflows to identify barriers and optimize the AD Initiative.

Qualifications

Your qualifications should include: 

Master’s Degree in Nursing required. At least 5 years’ experience in value-based care, population health, quality/process improvement, case management/nursing navigation, and project management.Direct value-based care/population health/payment and care transformation experience. CITI Training – Human Subjects Protection Course: Social and Behavioral Research, required.CITI Training – Health Information Privacy and Security (HIPS), required.CITI Training – Good Clinical Practice (GCP), required.Certification in relevant area: e.g., OCN, ACM-RN, EBP. Required.RN license preferred.Value-based Care, Population Health, Case Management, and Nursing Navigation, Value-base Care, preferred.
