Job Summary

Responsible for the coordination of the Long Term and Community Integration Services (LTCIS). Provide supervision, training, and guidance to assigned staff. Develop and implement Individual Service Plans (ISP) and provide support to assigned participants. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and (Commission on Accreditation of Rehabilitation Facilities) CARF, as well as other funding sources and company policy and procedure. Hire, train, and evaluate assigned staff and perform the duties as assigned by the Division Manager or designee.

Primary Duties

Complete and review in an accurate and timely manner all evaluation/assessments/annual plans, progress reports, daily notes, incident reports, contacts, reviews, attendance, and other supporting documentation.Monitor services being provided by assigned personnel and ensure that the defined needs of the individuals served are being met.Supervise staff to ensure that comprehensive and appropriate services are provided to individuals being served in conjunction with task specifications and supervisory instructions.Ensure all programs and documentation comply with any and all regulatory agencies. Maintain a safe, secure and healthy work environment for all participants, staff, volunteers, interns, contractors, and visitors.Report and document any unsafe conditions to the appropriate agency/individual. Maintain familiarity and ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy and procedure. Ensure that all individuals adhere to the ServiceSource practices, guidelines, and standards.

Additional Responsibilities

Assure that records of participants are being maintained to meet all regulatory standards. Provide support to the individuals and group as needed.Meet with representatives of other community agencies, families, and significant others for employee program planning and review. Complete and pass the Orientation to Developmental Disabilities (ODD) test and competencies. Coordinate and participate in Person Centered Planning (PCP) process and other meetings pertaining to the participant’s outcomes and objectives.Ensure the development, writing, and implementation of the PCP. Maintain familiarity with Behavior Intervention techniques.Attend county Behavior Management and/or Human Rights Committee to present behavior intervention programs for participants served, if needed.Coordinate services with the support coordinators to facilitate total program services. Provide support in areas of program design, development and implementation.Complete and/or insure employees complete in an accurate and timely manner all evaluation/assessment reports, progress reports, case notes, incident reports, attendance and other supporting documentation.Maintain up-to-date information as required for participant files and funding sources. Ensure that the needs of the participants are being met and monitor services being provided by assigned personnel.Maintain participant files in compliance with Medicaid, Licensing, and CARF standards, etc. Coordinate with a wide range for resources, both internal and external, to provide meaningful services and activities for all participants. Monitor employee’s compliance to organizational policies and procedures.Establish and maintain a working relationship with assigned employees, support staff and the members of other departments.Perform other responsibilities as assigned.

Qualifications: Education, Experience and Certifications

Required: High school diploma or General Educational DevelopmentPreferred: Bachelor's degree in Human Services or another related field preferred.Required: Three (3) years’ experience in the human service field.Required: One year (1) supervisory experience.Must pass State of Virginia Department of Behavioral Health and Developmental Services (DBHDS) fingerprinting and criminal background check.Must pass the ODD (Orientation to Developmental Disabilities) assessment and competencies.Employees who drive individuals in the community or on behalf of the company must possess a current and valid driver's license. Eligible drivers must have a good motor vehicle record (MVR). Driving requirements vary by site.If eligible to drive, must pass the online driving training within 15 days of hire.

Knowledge, Skills, and Abilities

Physical ability to support participants with personal care (feeding, toileting, transferring, etc.). Ability to relate well to people at all levels of an organization.Proficiency in Microsoft Office skills, and the ability to become familiar with corporate specific programs and software, such as Microsoft Teams or ZOOM.Ability to solve practical problems and adapt to new information and guidance quickly.  Excellent verbal and written communication skills.  Detail-oriented and able to carry out work with the highest levels of accuracy.  Ability to work independently and as part of a team. Knowledge of behavior intervention principles.  Knowledge of community resources.
