Program Manager (GUIDE Model)

Position Overview : The Program Manager (GUIDE Model) is responsible for implementation of the GUIDE Model in all markets while working collaboratively with the Director to ensure alignment with company strategy and forecast. The GUIDE Program Manager is responsible for the effective implementation and day-to-day operations of the GUIDE Model across assigned markets. The Program Manager oversees the Care Navigators throughout the organization, providing support and guidance to ensure proper care delivery. This position will establish the tools, processes, and systems to optimize the role of the Care Navigator. The Program Manager is expected to work effectively across departments and collaborate with colleagues to carry out day-to-day operations. She/He will aim to develop talent within the team, including acquiring, engaging and retaining Care Navigator staff. Schedule: Full-time position. Hours (with some flexibility on start/end times) are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays. Location: Up to 25% local travel is required for this position. The remaining time is spent working  remote (work from home).  Salary: $80,000 - $90,000. Base salary will be commensurate with experience.
