PREFERRED KNOWLEDGE:
The ideal candidate should have knowledge and experience with housing resources and placement and the ability to collaborate with and build relationships at the community level with both subsidized and non-subsidized community rentals to assist in building resources for field staff.
OAAS MY PLACE LOUISIANA (MFP) TRANSITION RESOURCE SPECIALIST: PROGRAM MONITOR
OCCUPATIONAL SUMMARY:
Position is domiciled within the Office of Aging and Adult Services State Office Division under Research and Quality as a part of the Money Follows the Person (MFP) Demonstration also known as My Place Louisiana. MFP is a high-level, Medicare and Medicaid (CMS) 100% federally funded, nationally published program with oversight provided by both the federal CMS and Congressional Committee. The MFP rebalancing demonstration is statewide and continues to assist Louisiana in adding the needed infrastructure and continue its efforts in rebalancing Medicaid long term care supports and services.
Position will provide coordination and monitoring in assigned LDH Regions and serve as back up to remaining LDH Regions. This includes monitoring resource initiatives which include, but are not limited to; housing, food bank/pantries, identity documents, proof of income documents, etc., communicating to state and regional office staff and to provide transition-related resources to either staff or contractors involved in transitioning individuals from nursing facilities.
Position will report to the My Place Program Monitor Supervisor (Transition Resource Manager) who oversees all resources related supports and operations statewide. This position will assist My Place Louisiana Transition Coordinators with identifying transition resources, coordinating requests for state issued ID cards, Birth Certificate, and income verifications and identifying other resources. Incumbent will possess knowledge of state and federal Medicaid guidelines and agency policies and procedures. In addition, incumbent will possess knowledge of the Medicaid service delivery system for long term care services, as well as knowledge of Older American's Act and Medicare funded services in the community. Position will adhere to pertinent laws, rules, regulations, policies and procedures of LOH, OAAS, Civil Services and appropriate state and federal licensing and governing bodies

Minimum Qualifications
MINIMUM QUALIFICATIONS:
Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

ESSENTIAL DUTIES:
40% Transition Resource Coordination and Support
+ Conducts comprehensive assessments of participant needs and preferences to identify appropriate resources needed to transition. Monitors participants to follow up on identified resource solutions.
+ Ensures all forms and documentation needed to secure community resources are received and processed in a timely manner, meeting all resource/referral guidelines and deadlines.
+ Monitors transition challenges and identifies solutions for unique challenges such as limited access to utilities, septic systems, and internet connectivity.
+ Addresses barriers for rural area resource needs including limited housing stock, distance and transportation, and availability of support services.
+ Addresses barriers for urban area resource needs including higher cost of living challenges, diverse housing options, and increased competition for support services.
+ Identifies and monitors resource options to present to the participant to ensure a person centered focus on resource utilization preferences.
+ Assists the participant with navigating complex referral options to facilitate a successful transition.
Completes necessary applications for identified resources, and monitors participant referrals to community partners and/or programs.
40% Community Outreach
+ Identifies community resources and communicates with supervisor on new community partnerships that provide supports to those transitioning back into the community.
+ Advocates on behalf of clients to ensure access to necessary community supports and resources, including housing subsidies, rental assistance, and public benefits.
+ Presents demonstration information to community partners and to relevant agencies.
+ Utilizes housing policy knowledge representing OAAS at all agency housing related meetings including, but not limited to Permanent Supportive Housing {PSH), Local Housing Coalition Agency meetings, and Louisiana Housing Corporation meetings.
+ Identifies and maintains a current log of transition resources and builds collaborative relationships with community partners to facility participant transitions. Identifies and monitors resource and referral opportunities.
10% Program Integrity
+ Provides training and in-serviceto regional employees on best practices and procedures related to resource utilization to ensure successful transition of the participant into the community.
+ Monitors post transition support and services on those that transitions needing housing including landlord communication, training and support on payment of utilities, follow up on any support requests.
+ Monitors current policies and or initiatives to apply to current application/processes as well as to make suggestions to current policies.
