Program Navigator Parents as Teachers (PAT) - Community Health Work Outreach

Northridge Hospital MedicalCenter is seeking a passionate and dedicated Program Navigator to join our Parents as Teachers (PAT) program. We are committed to providing high-quality, home-based services that support parents in nurturing their children's healthy development from birth to kindergarten entry. This is a rewarding opportunity to make a significant difference in the lives of families within our community. This is a grant-funded position.


Responsibilities
Actively participate in outreach efforts to recruit eligible families, building relationships with community partners (e.g., hospitals, health clinics, social service agencies) to identify and refer families. Conduct initial home visits to introduce the PAT program and assess family needs.Manage the program schedule, coordinating home visits for PAT Educators and ensuring timely delivery of services. Maintain accurate and up-to-date records of family participation and progress.Collect and input data into the program database, ensuring accurate tracking of key performance indicators (KPIs) related to family engagement, program participation, and child development outcomes. Prepare regular reports for program supervisors and grant funders.Provide administrative support to the PAT team, including scheduling meetings, managing supplies, and assisting with program events and training. Build and maintain strong relationships with community partners to ensure seamless referrals and support for families. Connect families with appropriate community resources and services to address their needs, acting as a liaison between families and other agencies.Contribute to the ongoing improvement of the PAT program by participating in quality assurance activities and contributing to program evaluation efforts.
Qualifications
High school diploma or equivalent requiredBachelor’s degree in social work, child development, or a related human services fieldExperience in community outreach, and knowledge of community-based organizations, services, and programs.
Experience working with families from diverse and high-risk backgroundsStrong computer skills and experience in Google Workspace.Strong data entry skills, database management, and ability to learn software programs implemented at the organization.Exceptional guest relations skills.Bilingual in English/Spanish preferred with the ability to communicate in the language of the community being servedAHA BLS is required within 90 days of hire. (CT CPT BLS)LA County Fire Card upon hire. (must obtain fire card 90 days from start date)Valid CA Driver’s License acceptable driving record and valid car insurance
