JOB SUMMARY: 

The Program Supervisor is responsible for ensuring the provision of services, assisting with the oversight of administrative processes, and supervising of staff/interns in the Helping you care navigation and coaching program. This program assists people facing a wide range of social determinants of health problems. The Supervisor is responsible for outreach, assigning, monitoring new enrollments to the program, and ensuring that the clients are appropriately connected to services.

ESSENTIAL TASK:

Provides oversight of the day-to-day operation of the program, including assessment, care navigation, planning, clinical interventions, and closures Conducts outreach activities to connect participants with services and monitors engagement Refer clients to internal healthcare services and other external referral partners as needed Provide relevant trainings to staff (e.g., co-occurring disorders, motivational interviewing) Model and coach staff in recovery-oriented, trauma-informed, and culturally competent care Provides supervision and completes evaluations for Staff’s, Community Health workers, Peers as assigned. Maintain relationship with sponsoring educational institutions of interns Maintains essential data for reporting and program evaluation in including program enrollment, progress notes, PPH assessments, referral activities and updating contact information in HYPHEN. Promptly completes daily, weekly, monthly and ad hoc reports i.e. (timesheets, evaluations, invoices/Billing, etc.). Build relationships with internal programs and services and act as a liaison between external referral sources and internal programs Conduct Quality Assurance documented review and audit ensuring compliance with ICL and funder standards Train new staff on the use of the electronic health record as it relates to standardized clinical workflows and billing Participate in the Interviewing and selection of new staffs/interns  Participates in regular supervision with assigned manager. Provide direct services and crisis management as assessed and necessary. All other tasks, or projects, as assigned by supervisor.

ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:

​​​Ability prioritize, think logically and follow procedures and instructions. ​Ability to support ICL mission, vision and values, standards and the code of ethical behavior. ​Ability to effectively communicate and interact with clients, staff, colleagues, public, other agencies and their staff and third party insurance personnel. ​Ability to work with clients, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences. ​Ability to prepare accurate and timely documentation, reports and other written material as assigned using computers and computer software assigned (e.g., HYPHEN, MAPP, etc.) ​Ability to understand and adhere to internal and external laws, rules, and policies ​Ability to work independently, and to conform to all applicable safety and accountability measures​

QUALIFICATION AND EXPERIENCE:

Associates or Bachelor’s Degree in social services, psychology, social work or other related field plus a minimum one (1) year experience providing case management or direct client service. Strong written documentation and communication skills. Good problem-solving skills and ability to work autonomously. Microsoft Office Suite, particularly Excel, required. Experience utilizing EMR systems.
