The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.

Required Qualifications: 

1 year of Job related experience in a healthcare setting.

Preferred Qualifications:

Associate's Degree Bachelor's Degree Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire. Prior experience in discharge planning, billing, utilization review, financial counseling or registration. Proficiency in medical terminology.
