Project Manager Community Health

If you are committed to social justice, health equity, and prepared to deliver care in new, innovative ways, you belong with us. _For the health of our community ... we are proud to be a tobacco-free campus._ **Responsibilities** The Project Manager for Community Health will oversee the planning, implementation, and evaluation of community-based health programs aimed at improving public health outcomes. This role involves coordinating with local healthcare providers, government agencies, and community organizations to ensure effective service delivery, managing project timelines and budgets, and facilitating communication among stakeholders. The Project Manager will also be responsible for assessing community health needs, developing strategies to address health disparities, and ensuring programs align with public health policies and goals. Additionally, the role will involve monitoring program progress, collecting data, and reporting on outcomes to stakeholders and funding bodies. + Leads/manages projects timely, clearly and concisely.\ + Develop and maintain a detailed project plan, including timelines, milestones, budgets, and resource allocation. + Define projectscope, objectives, and deliverables in alignment with grant requirements. + Manage expectations and relationships between multiple departments and community organizations. + Facilitate project meetings. + Coordinate the collection, assimilation, and analysis of information from project team members as neededand provide actionable information for projects and/or operational decision-making. + Prepare and presentregular projectstatus reports to leadership and stakeholders. + Serve as point of contact for communication, updates, and information flow related to project. + Manage the preparation and submission of all required grant and program reports, ensuring accuracy and timeliness. + Able to work independently, demonstrates critical thinking,system-oriented solutions, and generally able to be a self- starter working on larger goals without day-to-day instruction. + Sustain or close out the project, ensuring all deliverables are completed and documented. Full-time employees can receive up to $18,000 over five years, while part-time employees can receive up to $9,000. **Qualifications** Minimum: + Project Management Professional (PMP) certification or Bachelor's degree in related field. Preferred: + Community Health Worker Certification + DEEP Certification + 1-3 years of project management experience in a community health and/or hospital setting. + Experience managing grant-funded projects **Pay Range** $29.16 - $42.28 /hour We are an equal opportunity/affirmative action employer.
