Providence Place - MDS Coordinator

Providence Place, a Lifespark Skilled Nursing Community, is looking for dedicated and passionate MDS Coordinator to join our growing team.

Providence Place is conveniently located on the bus route, located in southern Minneapolis in the Standish Ericcson neighborhood. Here we provide services for our seniors in a TCU (Transitional Care Unit) Care, Long-Term Care and Memory Care. We have been helping seniors Age Magnificently for over 25 years and are part of Senior Care Communities, a local non-profit supporting senior.

Schedule Available: Full Time
Wage: $44-$47 based on experience

In this position you will be responsible for overseeing and facilitating the successful scheduling, completion, and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. The MDS Coordinator will monitor coding and documentation to ensure accuracy and conducts and coordinates ongoing resident assessments necessary for the accurate completion of the Minimum Data Set (MDS), Care Area Assessments (CAA), and the resident’s “Plan of Care”. Duties include:

Educate nursing staff on proper documentation to ensure quality of care, compliance, appropriate assessments, and interventions including POC training for nursing assistants and PCC clinical nursing assessment.

Ensure consistent, accurate and timely completion of the RAI process which includes MDS, Care Area Assessments (CAAs) and plan of care.

Work with the Interdisciplinary Care Team in developing a comprehensive resident assessment and care plan for each resident.

Verify required documentation is accurate and State and Federal reimbursement guidelines are met.

Determine Medicare/Managed Care eligibility on all admissions and readmissions.

Determine charting needs for Medicare/Managed Care Skilled covered residents, ensuring this information is available to nursing staff.

Coordinate scheduled Medicare/Managed Care meetings with other departments regarding continued coverage of Medicare A and Managed Care residents.

Review the status of continued coverage for Medicare A/Managed Care residents, including determination/coordination of in-lieu of days, MSHO days, etc.

Coordinate completion of Medicare denial letters with accurate information and inform the responsible party/ coordinate appeal process as necessary.

Preferred Qualifications:

Valid state Registered Nursing licensure.

Minimum of one year experience in long-term care with a strong background in the MDS and regulatory compliance preferred.
