About the Role: Behavioral Licensed Practical Nurse (LPN)
As a Licensed Practical Nurse, you will specialize in providing nursing care to patients with mental health, emotional, or behavioral conditions. Your responsibilities often combine traditional nursing tasks with behavioral health support to promote patient well-being. In this role, you will go into homes to support the patient i their journey.
Schedule: Monday through Friday, 8:00a to 4:00p
Territory: Springfield Greater Area

On an Average Day, you will:Ensures patient and staff confidentiality per HIPAA regulations.
Incorporates patient care goals into nursing visits, documenting appropriately.
Communicates significant changes in patient condition.
Implements patient safety plans using available resources.
Suggests revisions to care plans based on patient needs.
Educating Patient and families.
Delivers skilled nursing care under RN or physician direction.
Documents medication regimen and updates profiles as needed.
Aims to prevent hospitalizations and ER visits through proactive care.
Maintains professional and supportive communication.
Performs additional duties or projects as assigned.
Qualifications:Active LVN license in good standing (as required by state)
At least 1 year of experience in an acute care or similar setting
Current CPR certification from the American Heart Association or American Red Cross
Ability to lift 50-100 lbs, move patients and handle equipment
A positive attitude and commitment to providing quality patient care
Excellent communication skills
Reliable transportation to perform job responsibilities
