**Psychiatric Nurse** **Job Details** **Job Location** WHCG - Milwaukee, WI **Description** **Job Purpose:** This is a technical position that provides holistic nursing care for persons involved in the programs they support. This is a highly skilled position involving the ability to assess and treat responses to actual and potential mental and physical health conditions, to promote wellness and refer persons to appropriate medical care. **Reporting Relationships** **Reports To** : WHCG ACT/CSP Manager **Direct Reports** : No direct reports **Essential Job Functions:** _(Reasonable Accommodations may be made to enable individuals with disabilities to perform these essential functions.)_ + Understand and demonstrates through practice, the treatment guidelines specified in DHS 63 (Community Support Programs for Chronically Mentally Ill Persons). + Provides support and administers medications as needed, for clients in the community and in their home, designated residence, or facility. + Assists clients in medication management and coordination, including the maintenance of medication record, reconciliation of medications, documentation of medication assistance needed, collaboration with MD and pharmacy on medication changes/reviews, ensuring clients medications are being provided to client as ordered. + Medication set ups for clients who have RN medication set up as a part of their recovery plan. + Collaborates effectively with internal and external systems to assist individuals served in achieving their treatment and recovery goals. + Provides comprehensive assessment, re-assessment, care planning, direct nursing care interventions, and termination of care within program guidelines and professional practices. + Accurately documents services maintaining a thorough client record following program policies and procedures within stated guidelines and timeframes. + Provide consultation and training to program staff regarding health-related issues. + Notify the case manager, appropriate program supervisor, and or psychiatric provider, of any psychiatric symptoms or behavior change that may require intervention and any health-related documentation for the client record. + Develop strategies for providing patient education and medication monitoring. + Provide training to staff on appropriate policy and procedure for medication management within the program areas. + Monitors and reports medication errors to appropriate person(s) as deemed necessary. + Ensures safe storage of clients’ medications, according to State regulations. + Attends and participates in team meetings, all staff meetings, in-service training seminars and conferences as required. + Providing Preceptorship supervision for student interns as appropriate. + Crossed trained to all program areas, and able to cover if departmental needs deem necessary. + Additional duties as assigned within the ACT team. + Provide transportation in your personal vehicle to clients, as team need dictates or as requested by supervisor or treatment team. **Required Education, Experience, Certifications, Licensure and Credentials:** _(Where appropriate, education and/or experience may be substituted)_ **Minimum Required Education:** Associate’s degree is required in the field of Nursing. **Minimum Required Experience** : Must have at least 1,000 hours of clinical experience working with working with adults with severe and persistent mental illness. **Required License – Certification – Registration** : Current CPR certification or obtained within the first 90 days of employment: Medical Terminology. **Required Valid Driver’s License:** Motor vehicle record acceptable to program and/ or contractual requirements. A valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage”. **Knowledge – Skills – Abilities:** + Knowledge of mental illness, substance use disorders, psychopharmacology and evidenced based practices specific to needs of the clients. + Awareness of personal, cultural, educational, and environmental aspects of community living for individuals living with a mental illness or substance use disorder. + Knowledge of biological, psychological, and social problems and appropriate therapeutic interventions. + Knowledge of psychotropic medications and side effects. + Knowledge of community health, social service, and advocacy resources available within the community. + Must be familiar with Microsoft Word, Outlook, Teams, and Excel. + Professional workplace appearance and conduct; friendliness and courtesy to consumers, coworkers, and vendors. + Knowledge of community support programs in Milwaukee County is desirable. + Ability to communicate clearly and concisely in English, both orally and in writing. + Familiarity with the Electronic Health Records (EHRs) system is desirable. + Access to a personal vehicle. + This position must be able to effectively communicate complex ideas and instructions to a variety of people from different backgrounds in both writing and verbally. + Diligently attends to details and pursues quality in accomplishing tasks. + Builds and maintains customer satisfaction with the products and services offered by the organization. + Partners with clients to identify and resolve complex or sensitive issues. + Adheres to all workplace and trade safety laws, regulations, standards, and practices. **Physical Requirements, Visual Acuity, and Work Conditions:** **Physical Requirements** :On occasion must be able to push/pull/lift/move objects weighing at least 50 pounds. Frequently or constantly lift, carry, push, pull, or otherwise move objects, including the human body. Possible exposure to blood and body fluids with some exposure to animals (pets), hazardous materials and infectious diseases. **Visual Acuity:** The worker must have close visual acuity to perform activities such as preparing and analyzing data and figures; transcribing; viewing a computer terminal. **Working Conditions** : This position may also work in a variety of indoor and outdoor work environments in the community which may include various outdoor weather conditions, loud noises, fluctuating temperatures, and the occasional use of stairs. This position works with office equipment including, but not limited to, personal computers, photocopiers, fax machines, and multiline phones.
