At Seacoast Mental Health Center, we provide hope and promote recovery for children, adolescences, adults and their families in the Eastern half of Rockingham county.
Candidate must be a registered nurse with the State of New Hampshire. CPR and Firs Aid training is recommended. Psychiatric Nursing experience is preferred.
Duties include:
Oversee ACT medication program
+ Administer Injections and Oral medication which can occur in either an office or community setting.
+ Assist patients with medication planners. This will include obtaining the medication, filling the weekly planner with or without the patient or coordinating with the pharmacy that is filling the patient’s medication planners. This would include the supervision and occasional delegation of medication delivery by the case management staff.
+ The ACT nurse will work closely with the ACT psychiatrist to help assess mental status and effectiveness of the medication. If changes to the medication regime are needed the RN will ensure that the changes are implemented.
+ Provide education around indications and side effects of medications to patients.
+ The ACT nurse will facilitate the delivery of medications to clients in the community who require daily, supervised medication administration. This duty may require early evening hours.
+ The ACT RN will monitor the documentation of all medication services.
+ Assess overall physical health of patients on ongoing basis. Provide appropriate basic medical assessments and basic nursing interventions and/ or make referrals to a community provider. Duties may include taking patients to medical appointments as needed.
+ Provide education and information to ACT patients, family and staff about mental illness, medication, side effects and physical health.
+ Attend daily team meetings and weekly supervision with ACT psychiatrist to coordinate care and address treatment or administrative issues.
+ Provide on-call crisis intervention and back-up according to team policy.
+ Be familiar with other nursing jobs at SMHC sites (Fairweather Lodge, Exeter office) and be able to cover RN duties as necessary.
