I. Major Responsibilities:
1. Under the supervision of the lead coordinator conducts telephone screenings according to established program policies and procedures. Accepts telephone, fax and email referrals from PCP offices, other agencies, etc.
2. Responsible for patient intake process including, but not limited to registration functions, insurance verifications, co-payment process, and scheduling.
3. Assists lead coordinator with development and maintenance of patient master schedules.
4. Gathers necessary medical record and medication information as appropriate and forwards to appropriate clinician.
5. Makes appropriate referrals to community agencies according to established guidelines.
6. Maintains case file documentation both in electronic and paper format documents and numbers each case in department electronic database.
7. Maintains project database, run period reports from database as requested.
8. Maintains and updates database for intakes and scheduling.
9. Provides weekly or as needed statistical reports to program director.
10. Maintains and updates educational material, brochures, booklets and patient handouts.
11. Maintains and distributes psychiatrist on call calendar.
12. Mails packets of patient education material packets to families, care givers, etc.
13. Performs administrative functions for program including copying, faxing, mailing writing letters, editing department printed material, flyers and orders program supplies
14. Covers phones and general duties for the consultation project as required.
15. Organizes and schedules staff and educational meetings.
16. Assist in development and maintenance of program’s policy and procedures including any materials that may be used.
17. Ensures highest level of customer service and satisfaction.

II. Position Qualifications:
License/Certification/Education:
Required:
1. High school diploma or equivalent with additional specialized courses required.
Preferred:
1. Associates Degree, in Psychology or Human Services desired.
Experience/Skills:
Required:
1. Minimum of three years of related work experience.
2. Knowledge of medical terminology, health and wellness and healthcare office experience required.
3. Ability to work independently while prioritizing workload. Excellent verbal and written communication skills required. Strong attention to detail and ability to maintain confidentiality.
Preferred:
1. Demonstrated expertise utilizing various software applications, including Microsoft Office applications, word processing, spreadsheet, presentation graphics, and database utilization preferred.
2. Medical system software, IDX, Allscripts, and insurance verification software strongly preferred.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
