Job Description:Provides therapeutic services and diagnostic services to patients and families. This is to include but is not limited to individual, group, and family therapy, mental health intakes and assessments, diagnostics, and coordination of treatment services for the patient. May be required to provide Minnesota DHS Clinical Supervision requirements as required for Partial Hospitalization Programs.

Education Qualifications:

Practice Details/Job Description:

1.0 FTE, full-time position with FT benefits; Monday-Friday 8a-5pBased in Park Rapids OR Detroit Lakes with potential outreach to Walker and Menahga clinics.Case load would be both Adult and Peds outpatients (no inpatient responsibilities)Work within an interdisciplinary team of primary care providers, psychologists, LICSWs, NPs, and psychiatristsMental Health Training, MSW requiredManagement of psychosocial aspects of chronic and acute diseases.Application of behavioral principles to address lifestyle and health risk issues.Consultation and co-management in the treatment of mental disorders and psychosocial issues

Job Functions:

In collaboration with the primary care provider, provider would treat and manage mental health and psychosocial issuesConducts client intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care provider concerning the clients’ treatment goals and planProvides consultation to the primary care providers to enhance their skill and effectiveness in treating mental health problemsGives primary care providers timely feedback the client’s about care, treatment recommendations and progress via documentation in the client’s record and verbal feedback;Assists, to the extent feasible, in the clients’ community functioning by helping with public benefits, vocational rehabilitation, social support, housing, etc.Appropriately documents the clients’ progress and diagnostic information in the treatment chart to sync with EMR interface and billing

 
Candidate Qualifications:

Has excellent working knowledge of evidence-based treatments for medical and mental health conditions.Has ability to work through brief client contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.Is comfortable working with an interdisciplinary team, and has strong communication skills.Has the ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.


Education/Licensure/Certifications required:

Graduate of an accredited LICSW program and licensed/ability to obtain license in state of MN, willing to obtain ND license within 6 months.Successful completion of Essentia Health credentialing process prior to practice.

Salary

Minimum $32.98 , salary will commensurate with experience. Potential to earn more with production.
