PT Client Advocate, Resources for Inclusive Support and Empowerment (RISE)
Who We Are:  CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive.  We take a comprehensive approach by offering more than 180 integrated programs in:  Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach more than 73,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are in living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today. CAMBA’s Resources for Inclusive Support and Empowerment (RISE) Program aims to reduce transmission of HIV, with a focus on men who have sex with men of color (MSMoC), including those who identify as transgender or gender nonconforming (TGNC). RISE will address health promotion through social media campaigns for MSMoC with messages about relevant health issues and preventive health strategies. RISE will also address the social determinant of health (SDOH) social isolation by creating safe, positive spaces for MSMoC to develop meaningful social connections and benefit from the support of peers in overcoming emotional, social, and health issues. The program will conduct support groups for specific LGBTQIA+ populations and host public events open to all LGBTQIA+ individuals in the community. Lastly, RISE will navigate clients to LGBTQIA+-friendly resources in the community with a focus on HIV prevention and legal services that address domestic violence, workplace discrimination or harassment, housing discrimination, and tenancy issues. RISE will provide services to 450 participants. Position: PT Client Advocate Reports To: Assistant Program Manager Location: 1259 Flatbush Avenue Brooklyn, NY 11226 What The PT Client Advocate Does: + Plan, coordinate and facilitate social/peer support events, including group facilitation for clients. + Prepare marketing materials for the program. + Create and post content for social marketing campaigns addressing HIV prevention and other health concerns for MSMoC. + Conduct outreach activities at high-risk venues to market the program in order to recruit new clients and re-engage former clients. + Develop action plans with clients enrolled in navigation or support services + Follow-up with clients and with referral organizations regarding client contact and progress with referral organization. + In collaboration with program staff, work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency. + Contact clients via telephone, mail and/or face-to-face home and office visits to ensure adherence to program, and document via progress notes. + Escort clients to appointments (educational, medical, social service, etc.). + Provide all required information for weekly/monthly/quarterly/annual reports. + Create and maintain client files and make copies of all client documents. + Prescreen clients over the telephone for eligibility and may schedule intake appointments. + Assist clients in completing all CAMBA intake applications and forms. + Schedule appointments for client with referral organizations. + Input client data and client progress information into automated database. + Participate in evaluation activities, such as conducting surveys with support group and event participants. Minimum Education/Experience Required: + Sufficient education (English-speaking, reading, writing, & math abilities) to comprehend oral and written information and to assist callers and visitors and staff. Other Requirements: + Two (2) years of relevant experience, preferably in a mid-sized (250+ employees) organization. + Good written & verbal communication. + Computer literacy in Microsoft Office Suite. + Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues. + Experience and/or comfort conducting online outreach via social media and dating/”hook-up” apps and websites. + Required to work occasional evenings/weekend days + Bi-lingual English/Spanish preferred Compensation: $21,583 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
