PT PARENT AND CHILD SOCIAL WORKER (VISITING COACH)

 

working collaboratively with a Team of Case Planners and Supervisors to provide assessment, establishment of goals and interventions to reduce the length of stay in residential foster care

provide outreach, referrals, escort and advocacy to connect families with resources, to increase economic stabillity and prevent housing instability

Assist families to complete applications and documentation as needed

completing initial psychosocial assessments for all new admissions within 30 days of placement

schedule and participate in all transfer FTCs for new admissions, 30 and 90 Day FTCs, and 6 month FTCs as assigned

providing Visit Coaching Services to support productive parent child visits between birth parents and their children;

supporting overall parent/child interactions in family visiting spaces- both as a direct support to birth parents and and as a resource to case work staff;

ensuring each visitation plan has measurable goals that the family is working towards

Applicants should have experience in providing counseling and social work services to children and parents, and preferably have experience with family therapy. They should also have strong experience working collaboratively across programs. Experience within a foster care setting  and/or working with a developmentally disabled population is preferred.

Flexible scheduled required, primarily afternoon and evening hours, with occasional weekend hours.

PT 21 hours per week.

Qualifications - External

Education/Degree: Master of Social Work Degree, LMSW preferred

Experience working with families and children, and working with an intellectually disabled population preferred.

NYS Driver's License required.

Candidates who are bilingual in Spanish strongly preferred.
