DESCRIPTION
The Public Health Professional III (PHP) for The Nleta Project is responsible for overseeing the implementation, coordination, and supervision of the holistic home visitation program aimed at supporting Black Infant Health Program Graduates. This role requires a licensed clinical mental health professional who will provide home visiting services, conduct assessments, develop personalized service plans, deliver health education, and provide counseling/therapy services to families. Additionally, the Program Manager will supervise the Nleta Project's Program Coordinator and Health Educator personnel to ensure program effectiveness and adherence to organizational standards.
EXAMPLES OF DUTIES
+ Implement and maintain a culturally affirming program that recruits, trains, and retains staff who reflect and respond to the values of the Black/African American community.
+ Conduct home visits to Black Infant Health Program Graduates and their families to provide comprehensive support and services.
+ Assess the needs, strengths, and challenges of families in a culturally sensitive and empathetic manner.
+ Collaborate with families to identify goals and develop individualized service plans to address their unique needs and promote holistic health and wellness.
+ Provide evidence-based counseling and therapy services to individuals, couples, and families to support mental health and emotional well-being.
+ Utilize a trauma-informed approach and culturally competent practices to address intergenerational trauma, stressors, and mental health concerns within the community.
+ Monitor client progress, evaluate effectiveness of interventions, and modify treatment plans as needed to ensure positive outcomes.
+ Develop and facilitate health education workshops, support groups, and community outreach activities focused on maternal and infant health, parenting skills, nutrition, and overall wellness.
+ Provide culturally relevant health information and resources to empower families to make informed decisions about their health and the health of their children.
+ Supervise and support the Nleta Project's Program Coordinator and Health Educator personnel in their respective roles and responsibilities.
+ Conduct regular meetings, provide guidance, and offer professional development opportunities to enhance staff skills and competencies.
+ Foster a collaborative and inclusive work environment that promotes teamwork, mutual respect, and cultural humility among team members.
+ Coordinate program activities, including scheduling, data collection, and reporting, to ensure compliance with grant requirements and program objectives.
+ Monitor program outcomes and evaluate the effectiveness of interventions using quantitative and qualitative methods.
+ Identify areas for program improvement and innovation based on data analysis and feedback from stakeholders.
+ Cultivate and maintain partnerships with community organizations, healthcare providers, and other stakeholders to enhance service delivery and expand program reach.
+ Participate in community events, meetings, and coalitions to raise awareness about the Nleta Project and promote collaborative efforts to address health disparities and social determinants of health.
REQUIREMENTS TO FILE
EDUCATION:
+ Bachelor's Degree in Public Health, Human Services, Business Administration, Public Administration or a closely related field (Proof of education required).
+ Professional experience beyond the minimum qualifications may be substituted for the required education on a year-for-year basis.
EXPERIENCE:
+ A minimum of one (1) year of full-time professional experience coordinating and/or supporting community-based social service or public health programs.
ADDITIONAL REQUIREMENTS:
+ A valid motor vehicle operator’s license.
DESIRABLE QUALIFICATIONS:
+ Lived experience in communities served by the Black Infant Health Program OR
+ Experience navigating similar social, health, or economic challenges.
ABILITY TO:
+ Engage community organizations and identify community resources serving the Black/African American community.
+ Demonstrate an understanding of the complex, interrelated issues and concerns contributing to health disparities affecting Black/African Americans.
+ Demonstrates cultural competence in implementing and maintaining a culturally affirming program.
+ Learn, interpret, retain, and communicate technical and complex information, terminology, policies and procedures related to grant programs and compliance.
+ Communicate clearly and concisely, both orally and in writing.
+ Maintain accurate records and files.
+ Work effectively with a wide variety of people by consistently exercising tact, good judgment and a pro-active, problem-solving focused communication style.
+ Operate office equipment including computers, printers and copiers as well as the word processing, spreadsheet, e-mail, database and program specific applications.
