The successful candidate will be responsible for conducting home health visits within the community, providing essential healthcare services to patients in their homes. Key Responsibilities: + Conduct home health visits to assess patient health needs and provide necessary care. + Develop and implement individualized care plans. + Educate patients and their families on health management and disease prevention. + Coordinate with other healthcare professionals to ensure comprehensive patient care. + Maintain accurate and timely patient records. + Participate in on-call duties as required. Qualifications: + Valid RN license in the state of North Carolina. + Experience in public health nursing or home health care preferred. + Strong communication and interpersonal skills. + Ability to work independently and manage time effectively. + Compassionate and patient-focused approach to care.
