Public Health Specialist Job Details Job Location Albany - Albany, NY Description Engage difficult to reach HIV+ individuals or those at high risk. Link and retain clients in health care and assist them to maintain medication regimens. Conduct confidential HIV/syphilis/HCV testing and STI screening. Coordinate with Public Health Support Specialists and other team members. Services provided throughout our 15-county region.Major Activities:1.Incorporate best practices regarding confidentiality into all job duties and communications in accordance with Article 27-F, HIPAA, Alliance for Positive Health policies and procedures and other applicable regulations. 2.Engage HIV+ and high-risk individuals who are not in care or sporadically in care. Employ techniques to actively support clients in activities that improve their health outcomes. 3.Address and remove barriers to enable client access to all necessary components of health care. If applicable, conduct home visits and/or meet clients at designated service providers and community locations to increase accessibility. 4.Conduct confidential HIV/syphilis/HCV testing and STI screening, as applicable, in venues serving targeted communities according to NYS DOH guidelines. 5.Work with Public Health Support Specialists to engage clients into care and treatment and support services. 6.Provide syringes to individuals identified as needing and refer to harm reduction services 7.Incorporate the Transtheoretical Model of Behavior Change, as applicable, into all aspects of programming. 8.Conduct multidisciplinary case conferences. Coordinate services and referrals with service and clinical providers that facilitate clients’ engagement and retention in care. Negotiate and advocate on behalf of clients for services that support self- sufficiency and self-management. 9.Maintain accurate records in accordance with New York state regulations and agency protocols. Maintain documentation and statistics on activities to comply with reporting requirements. 10.Perform other duties as assigned.Qualifications:•Associate’s Degree in Human Services, Public Health or related field.
