Role Summary: The QIDP will provide support the two ICF’s and will assist with QIDP needs within the Residential
team. They will ensure person centered plans are being followed. They will coordinate, monitor and documents services
provided will advocating for the persons supported.
Role Responsibilities:
Creates, revises and maintains CFA's and all required documentation in program books on a monthly basis or as
the need presents itself.
Prepares reports and other informational materials as needed.
Communicates with all appropriate parties about any emergencies or sensitive situations.
Completes and submits annual Medicaid Recertification and resolves issues affecting an individual’s
Medicaid/Medicare/private insurance.
Assists, as needed, in the management of behavior and emergency care.
Handles all daily and monthly paperwork for the house, including medical and billing, within the required deadlines
in adherence to related regulations or agency policies.
Ensures service design, which provides everyone with an appropriate active treatment program and supervises
and directs person-centered agenda.
Facilitates Life Plan meetings for the ICF people supported, and IRA locations if a need arises.
Completes monthly observations to confirm services are delivered as prescribed.
Ensures that there are adequate environmental supports and assistive devices to promote a person’s
independence and participation in community integration, based on recommendations.
Advocates and ensures that the people supported receive services corresponding to Personal Outcome
Measures.
Acts as a liaison between the Day Program, Residential Program, and families to provide consistency for those
supported.
