Responsibilities:

Deliver exceptional and individualized care as determined by each resident’s care planReport changes in resident health status to the clinical teamConnect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experienceEngage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiencesComply with all rules and regulations regarding confidentiality and privacy of team members and resident informationPerform any other duties as assigned

Qualifications:

Must hold certification indicating successful completion of Qualified Medication Aide (QMA) program in IndianaInsulin certification required at applicable locationsHigh School diploma or GED equivalent requiredExceptional communication and interpersonal skillsMust demonstrate the ability to serve with love, compassion , and dignity
