We are currently searching for empathetic Qualified Mental Health Professional's!
Job Scope :
The QMHP will be well versed in resource/navigation for comprehensive client services and follow-up. Responsibilities include skills training and psychosocial interventions to groups and individuals, follow-up calls, appointment scheduling, resource navigation, educating, as well as other duties as assigned. The QMHP will work hand in hand with the MD, LPHAs and RNs to provide the client with resources pertaining to diagnosis, services, medication, housing etc.
Key tasks may include:
· Completing ANSA and other standardized assessments to determine need for rehabilitation services as needed,
· Completing Crisis Assessments including the ANSA,
· Developing service goals/objectives in collaboration with the client,
· Participating in the development of the client’s Plan of Care in collaboration with the client and other team members,
· Providing small group/individual rehabilitative skills training according to authorized curricula,
· Assessing client’s progress in meeting service goals/objectives,
· Insuring client/family satisfaction with services, participating in updating POC as needed,
· Coordinating the completion of responsible staff activities,
· Completing required documentation accurately and in a timely fashion,
Other duties as assigned.
Education/Licensure :
· Current valid driver’s license in state of residence, proof of automobile liability insurance, and a driving record insurable by the center’s insurance carrier.
· QMHPI: Bachelor’s Degree in Human Services that meets the DSHS criteria for a QMHP
QMHPII: Masters’ Degree in Human Services that meets the DSHS criteria for a QMHP
