Job Description
Under the supervision of the Chief Quality Officer (System), the Quality Coordinator plays a vital role in our healthcare delivery system. Primary areas of accountability may include:
Creation and execution of hospital wide “Conversation Ready” InitiativePoint person for any ACP/EOL needs on patients without Palliative Care on boardSupportive role to all clinicians to ensure thorough ACPSupportive role to all clinicians to provide the highest level of care to patients nearing EOL.Ongoing training/involvement for designated “Conversation Champions”Creation of educational and interactive resources for patients and their families to assist with EOL decision makingIdentification of triggers to start palliative and hospice consultations earlier in the hospitalizationOngoing clinician palliative concepts educationConversation Ready educational classesSkills Day Conversation Ready educationDevelopment of Cerner documentation to capture ACPDevelopment of note templates to capture ACP conversations ? Continued process improvement/growth of the initiative Data collection on impact of the initiative Assist with goals of GIP hospice initiativeNurse educationNurse GIP conversion protocolHospice liaisonSupportive role to bedside nurses to provide highest level of care to patients under to hospice care
