**Description:**
The Quality Improvement Coordinator assist in leading the Quality Department at Saint Francis (SFH) and Mercy Fitzgerald (MFH) campuses to provide a high quality-high care for our patients and families utilizing our Quality Metrics (ECQM) cornerstones. The Quality Improvement Coordinator will assess, analyze, and recommend quality and clinical performance improvement processes to assure that the highest standards of quality care can be achieved. Will provide clinical expertise to assist with case-finding, identification of opportunities, and performance improvement activities. This position will also measure performance and see opportunities to enhance performance in value-based systems of care. As healthcare evolves with new models of service delivery, quality improvement activities will aim to ensure that care is both patient-centered and focused on maintaining the health of our entire patient population in an efficient and effective way. This position serves as a key participant in the successful management of value-based contracts, CMS ECQM metrics and focusing on both individual patients and population trends. The Quality Improvement Coordinator collaborates closely with clinical, operational, and administrative teams to promote a quality performance. Quality Improvement Coordinator will quality reports to various committees ensuring performance improvement goals. Data collection and analysis skills are integrated to be utilized in the performance improvement (PI) process, as well as other responsibilities of the position. The Quality Improvement Coordinator will (participate or Co)-chair/facilitate committees/teams as requested by the Quality Manager. The Joint Commission, CMS, and other regulatory bodies including regulatory compliance will be part of the Quality Improvement Coordinator position.
+ Analyzes, researches and designs new workflows and processes with clinical partners to improve performance and outcome metrics.
+ Monitors early warning indicators and dashboards and escalates concerns to clinical teams and recommends actions including education of colleagues as needed.
+ Participates in Performance Improvement activities to assure continuous Quality Improvement in conjunction with Leapfrog and Star Rating metrics.
+ Ensures the protocols and therapies become the standard of care followed within the facility by increasing utilization.
+ Works in partnership with clinical documentation improvement to improve documentation accuracy.
+ Demonstrates a high level of clinical competence that creates an environment of clinical excellence resulting in improved patient outcomes.
+ Provides coding and medical staff support to develop meaningful education strategies.
+ Delivers presentations to clinicians, leadership, and administrators.
+ Assists in maintaining action plans across the system and quality scorecards that measure improvements. This information is used to develop an annual report of accomplishments.
+ Builds sustainable relationships with provider sites to support quality documentation and education.
