Quality Improvement Manager

Purpose

The Quality Improvement (QI) Manager leads a team of QI Specialists to coordinate and execute QI activities across the organization to advance value-based payment and population health priorities. The QI Manager provides leadership in deploying tactics and resources to successfully meet the defined quality priorities and track progress toward those goals. This position will analyze data to identify and address care gaps, provide performance feedback and tools to clinics/programs, facilitate process redesign as appropriate, and manage certification program reporting. The QI Manager position will collaborate with key internal stakeholders, including the Patient Outreach & Engagement Team (POET) and clinical operations, as well as external partners to support population health initiatives designed to improve the quality of care and health of the communities Neighborcare serves.

In this position you will

Primary Responsibilities:

Manage a team of QI Specialists that supports quality priorities.  Oversee and monitor Managed Care Organization (MCO) data management, including performance tracking across contracts, providing data visibility to DQPE and CHO, producing actionable gap lists for key stakeholders, and conducting regular chart/data audits.  Manage grant & sponsored partnership programs tied to quality improvement priorities (g. Mobile mammography partnerships; American Cancer Society grants, etc.).  Coordinate and perform QI activities for quality improvement initiatives set forth by Director of Quality & Patient Engagement (DQPE) and Chief Health Officer (CHO).   Conduct ongoing analysis of data to assess gaps in and barriers to patient care, and identify action plans and opportunities to implement evidence-based protocols ensuring alignment of HEDIS and UDS.     Manage and complete certification program reporting (g. Patient Centered Medical Home recognition, American Heart Association certification, etc.).  Coordinate program/clinic-based QI performance tracking and facilitate quality improvement-related process redesign centrally and within clinics.  Develop care team training materials and lead or coordinate trainings as appropriate to meet QI needs of the organization.  Coordinate priorities for the Patient Outreach & Engagement Team (POET) Supervisor in partnership with DQPE to ensure alignment in QI and Population Health efforts using data to drive efforts.  Participate in clinical operations and Epic workgroups to ensure QI and population health needs are effectively embedded across organizational efforts.  Actively participate and be an organizational leader with MCO’s, including attending regular meetings and tracking all updates and needs.  Provide quantitative or qualitative information to the Grants team as requested.  Participates in additional meetings with external and internal stakeholders, as needed.  Perform other duties related to quality improvement and population health outcomes as assigned by DQPE.  Uphold Neighborcare Health service commitments: (a warm welcome; respect; caring; working together; and trusting relationship) with internal and external stakeholders. 

Key Skills, Knowledge & Abilities

Required For This Job:

Strong analytical and systemic thinking skills, with ability to synthesize information from multiple data sources to develop recommendations  Familiarity with HEDIS data sets, HRSA UDS, and population health principles  Expertise with continuous quality improvement, process mapping, root cause analysis, and developing standard workflows  Strong proficiency in QI principles, such as Model for Improvement, PDSA, and/or Lean, etc.  Proficiency in Project Management   Comprehension of value-based care and population health concepts, and experience in Medicaid/Medicare Managed Care  Ability to provide training and education to staff at all levels  Well-developed written and oral communication skills, including facilitating conversations and presenting  Knowledge of ambulatory electronic health records, preferably with Epic  Utilization of Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, SharePoint)  Proficient in using advanced Excel features such as pivot tables, complex formulas, and data visualization tools (charts/graphs)  Strong organizational skills to prioritize multiple tasks and projects under tight deadlines  Ability to effectively utilize or quickly learn various population health and clinical data report platforms, as well as comfort managing large datasets (g. Epic Reporting Workbench and SlicerDicer, SQL, DOMO, PowerBI, etc.)  High degree of adaptability, productivity, reliability, and ability to work independently. 

Preferred For This Job 

Experience in Federally Qualified Health Center (FQHC) or Community Health environments  Lean or Six Sigma experience 

Required Qualifications

Bachelor’s degree required  2 years of related experience in quality, performance improvement, or related field in healthcare

Preferred Qualifications

Advanced degree preferred, or RN

Compensation

The wage range for this position is a minimum $85,633.60/salary up to $119,995.20/salary maximum

Compensation will be determined based on years of relevant experience
